     Vicki Hayes completed her Masters Degree in Communicative Disorders in 1977. For the following 12 years she worked as a Speech and Language Pathologist solely in the acute, in/out-patient, as well as acute rehab. At that time, rehabilitation in the skilled nursing arena was lacking and Vicki saw a need to enhance the level of care especially in the area of dysphagia.  As a result, Rehab Alliance was born, started by both herself and her husband David, a physical therapist. Now nearly 30 years later Rehab Alliance maintains its strong reputation, serving both profit and not for profit organizations. The patients continue to be their passion, fueling the development of new and innovative programs to enhance functional outcomes in all areas of rehab.

     Dave Hayes and his Wife Vicki Hayes founded Rehab Alliance in 1987. Dave has vast experience in all setting both as a treating therapist and on the business/management side. Dave is the two time president of the California Chapter of the American Physical Therapy Associations (APTA). He has written and been involved in many of the current regulations that currently dictate physical therapy practice. 

     Dave is an expert in therapy services including management, regulations, ethics, business development, contracting, reimbursement structure, financial management and conflict resolution. 

Dr. Denese Kaufeldt Montano, PT, DSc, GCS, CEEAA

     Dr. Denese Montano has been a physical therapist for 26 years. In 1992 she received her BHs and MPT then in 1998 she received her D.Sc. from Loma Linda University. During her doctoral study she completed research on Low Back Pain and Treatment Classification for which she received the student research award for excellence in student research.  In 1999 she received her Geriatric Certified Specialist designation from the American Board of Physical Therapy Specialty and in 2010 she completed her Certified Exercise Expert for Aging Adult’s.

     Dr. Montano is a dedicated and patient-focused physical therapist with proven strengths in Regional Management, Staff Development, Rehab Team Management¸ Educational Development, Acute Rehabilitation, Inpatient Geriatric Rehabilitation and Outpatient Geriatric Rehabilitation. She has taught as an adjunct professor at Azusa Pacific University in the master’s of physical therapy program and at Loma Linda University in the advance doctorate of physical therapy program. In addition, she has been a guest speaker at medical conference’s and an expert witness.

     She is currently the regional director of Rehab  Alliance for therapy services for the Inland Counties District and the greater Los Angeles area and is serving as the chairperson for the inland counties district of the California physical therapy association. In her free time she trains in traditional Shito-ryu karate-do , Genbu-kai and is ranked as a second degree black belt in karate and in kubudo (okinowian weaponry).

 

 

 

Brooke Burki, DPT, OCS, RAC-CT

     Dr. Brooke Burki DPT, OCS graduated from Chapman University and is a Board Certified Orthopedic Specialist.

     As Chief Business Development Officer for Rehab Alliance, Dr. Burki's focus is implementation of resident and staff wellness, screening programs, expansion projects, internal and external marketing, census development and facility outreach. 

     Dr. Burki takes an interest in learning the individual culture of each facility and implementing strategic planning for continued growth. Aside from management and business development, Dr. Burki has experience with direct hands-on patient care in a variety of settings including; outpatient, acute and post-acute.  In her free time, Dr. Burki enjoys traveling, exercising and spending time outdoors with her husband and family.

Garrett Hayes, DPT, OCS, CSCS, RAC-CT, MBA

          Dr. Garrett Hayes, DPT, OCS, CSCS, graduated from the USC Doctorate of Physical Therapy program. Dr. Hayes is also a Board Certified Orthopedic Specialist and a Certified Strength and Conditioning Specialist. Garrett also has a Master's of Businesses Administration with an emphasis in Healthcare Administration. 

         As Chief Administrative Officer, Dr. Hayes' duties include company efficiency, productivity trends and management, technology management and contract administration.

         Dr. Hayes has experience working and managing as a Physical Therapist in acute care, sub-acute care, acute rehab, skilled nursing facilities, and outpatient rehabilitation. In his free time Dr. Hayes enjoys spending time with his wife, daughter and his two golden retrievers, Jake and Finley.

Claudia Yanez, DPT, graduated from Western University Doctorate of Physical Therapy Program.
Claudia has worked for Rehab Alliance for 10 years and along with patient care, brings a strong knowledge base of EMR usage, management, quality assurance, staffing, company policies/procedures and legal compliance.
